    A year ago if you’d said that an app could replace medication, you’d have been laughed at. In fact we were. But that’s changing, as even the NHS is recognising the potential that apps can make to a care strategy, rendering it more person centred than conventional treatment. Apps, such as RemindMeCare, impact on activity, music, cognitive stimulation and physical therapies using gamification and social engagement, whilst monitoring outcomes, to improve wellbeing.

    Vanguard Funding helped us try out a method of properly upskilling care practitioners who wanted to do so much more! The NHSE Vanguard Programme was an enabler for Herts Care Partners to look at something: we had believed would make a difference; scale it up; and prove it worked. We worked with 20 homes and put them through a stretching education programme, we then supported them with implementation and rewarded them for their efforts. This presentation will take you through our journey and help you to see how you can take our learning and implement it in your own area
